Signing up for FORMED.org:

1. Go to http://formed.org.
2. Click on the SIGN UP button at the top right of the page.
3. Select I belong to a Parish or Organization.  
4. Under Create A New Account, enter Holy Family Fort Worth TX and select our parish from the dropdown list.
5. Click Next.  
6. Enter your name and e-mail.
7. Click NEXT and you should have access.
